Dear Farmington Water Board Official(s):
My name is Moni Islam, my family and I reside at 20330 Erickson Path in Farmington. I
am writing to request a reconsideration of the water use citation we received on May 27,
2008. This is our first offense and the violation was the result of honest forgetfulness, not
an active act of disobedience to the City of Farmington.
We decided to test the set-up of our underground sprinkler system in conjunction with the
soaker hose in the boulevard on that evening so that my 6 months pregnant wife will not
have to worry about it while I was gone to an unplanned business trip out of town. We
turned the underground sprinkler system off immediately after but forgot to shut off the
soaker hose right away since we became distracted by our toddler in the backyard.
During this time, the citation was issued according to the paper we received in the mail.
We have always been obedient to City of Farmington and have no prior citation of any
kind in past five years of residence in Farmington. We sincerely hope that you would
reconsider and remove the citation.

Monthly Work Report

1. Check wells; daily
2. Daily water sample (fluoride); daily
3. Read misc. meters: move outs, move in's, rereads... daily
4. Most times doing steps l.2and 3 before break.
5. Install new meters, change out old with new
6. Do monthly water samples; these samples are done every first
Wednesday of the month, make copy of paperwork: one copy for
samples and one for our records. A chlorine sample is also done at this
time at every location, which are approximately eleven sites.
7. Read meters: start with manual and transmit route first then doing
Orion route next, consists of approx. forty hours or less depending on
weather conditions or problems that might arise. Route must be done
by the 15th give or take a day so that finance can finish billings.
8. Do multiple quarterly samples and send in, done at approximately 4-
29-08. Sent in on 5/1/08.
9. Grease and lube motors in well# 1 and #3.
IO.Start up irrigation systems at wells 5,6,and 7. Couldn't start well #4
because well was down for maintenance.
II.Start mowing well sites.
12.Change out acid lines and injector at well #4.
13.Fertilize well #4 with Seotts weed and feed.
14.Took off outside cover and checked A.C. unit at well #4.
